Completing the questionnaire qualifies you for entry into a prize draw to win 1 of 3 £50 Tesco vouchers. Draw date Friday 27th November 2026 - winners will be announced on Monday 30 November and vouchers given out WC 30/11/26.

Survey findings may be published in reports or presentations but individuals will not be identified without their explicit consent.
You have the right to request access to the personal information we hold about you, request corrections to inaccurate information, or request deletion of your personal information where appropriate.
